  1. TO ENTER:
    Complete the Submission Form below providing CityofAngel.com with all information requested. Fields left blank will result in invalid entries. One Entry per person/per household please, duplicated entries will be discarded from the drawing pool. The Give-away submission form will be open from Monday, February 2nd, 2004 until 8:00pm EST on Friday, February 6th, 2004, at which time the Give-away will officially close.

  2. NOTIFICATION AND PRIZES:
    All winners will be notified via email, phone, or regular mail. If a winner cannot be reached after a reasonable effort has been made during five (5) business days from the first attempt of CityofAngel.com an alternative winner may be selected.

    Please allow 3-5 weeks from the close of the Give-away for the delivery of any prizes won. The Prizes include:

    All winners will be selected at random from the completed submission entries drawing pool. All winners (first name only, city/state) will be posted HERE at CityofAngel.com on Monday, February 9th, 2004. All personal information will be kept confidential by CityofAngel.com and not released.
